In order to effectively assess a prospective student for placement in a program of Lakeside in a timely manner, the following information is needed when making a referral.
All Students
- Reason for referral
- Most recent report card and current grades
- Current class schedule
- Updated transcript for students in grades 9-12
- Attendance records (current and previous school year)
- Student Attendance Improvement Plan (SAIP)
- Discipline records including documentation of significant disciplinary and behavioral issues PSSA and Keystone Exam scores
- Health and immunization records (except for Mobile Support Program)
- PA Secure ID number
Special Education Students
- Current IEP including FBA(s) and Positive Behavior Support Plan(s)
- Recent Manifestation Determination(s)
- Most recent Evaluation Report (ER or RR)
- RR Waiver (when applicable)
If Applicable, the following information pertaining to all students is also beneficial.
- Academic testing and evaluation results
- Psychological and/or psychiatric evaluations
- Psycho-social information
- Court order or notification of placement
- Any other relevant information

When making a referral, if you have a specific school of Lakeside you want considered, please indicate which school(s). Should you have no preference and want Lakeside to recommend which of its schools can best meet a prospective student’s needs, you may indicate that as well.
